Preparing For Your First Visit

If time allows, we will mail you our medical information forms to be completed prior to the first visit. Please bring them with you. We would like you to complete these forms and bring them with you to your appointment.

Your informational packet will include:

  • Medical history form
  • Registration information
  • Insurance information (please bring your insurance card with you)

If you do not receive this information prior to your first visit, please plan to arrive 15 minutes early so that you may complete the forms. This information is extremely helpful to our physicians as it assists us in making a proper diagnosis.

You should also plan to have all medical records, including test results, sent to our office prior to your visit. You may also choose to bring them with you on your first visit. We also would like the names and addresses of your primary care physician and any other medical professional who has cared for you recently.

In general, first time appointments require approximately one hour, depending on whether diagnostic tests are recommended. Occasionally, medical emergencies cause delays in appointment times; we appreciate your patience and understanding if this occurs.

Your appointment may include:

  • A complete cardiac examination
  • EKG (electrocardiogram)
  • Stress test (treadmill)
  • Echocardiogram (ultrasound of the heart)
  • Stress Echocardiogram

It may be necessary for you to undress from the waist up. In this case, we recommend that you wear a shirt or blouse which is easy to remove. Dresses and one-piece outfits may be cumbersome.
